Second form for buying handgun?

I bought a handgun at my LGS last month and after filling out the 4473 form they gave me another Virginia form to fill out. I questioned it and they said this form has been required for years. I know I did not fill out another form when I bought a shotgun three years ago. I should have noted the form number but didn't. Does anyone know what this form is about and its number?
In NJ, the bad guys are armed and the households are alarmed. In VA, the households are armed and the bad guys are alarmed.

The second form is the Virginia State Police background check, which costs $2 regardless of type or quantity of firearms purchased. It's been around as long as I can remember.

Thanks OakRidge! Would anyone happen to know the form number? I searched the VA State Police site but cannot find a copy of the form. I just want to know what information I provided. I recall that I did not have any problem with the form at the time but would like to see it.

Interesting that, unlike with form 4473, a copy of the SP-65 form is submitted to the State Police. The Procedures Manual does not indicate what the State Police do with this form or how long they keep it. Not exactly firearm registration as the firearms are not identified by serial number, but getting close as it does show how many firearms of what type are purchased by whom.

This is a hold over from when VA implemented instant background checks before there was a NICS to run checks with.

IMO the Va State police running a duplicate background check system is redundant and should be eliminated.
