CMS contracts out damned near everything. The organizations that actually process the claims are called FI's, or fiscal intermediaries. More populous states will have more than one run by a different company. They have their own staff and to my knowledge none of them are union, anywhere.

Indeed. I saw a neurosurgeon rooting through my trash the other day, oh, wait, that will never happen. Affordable care act doesn't cut payments to doctors. That game is the annual Medicare payment adjustment that gets pushed through every single year, like the AMT it just never gets fixed.
